Andover Traveling Boys Basketball

The following forms and items will be collected at try-outs.

1. REGISTRATION FORM
   Bring a completed and signed Player Registration Form to try-outs. To be eligible to participate in Andover Traveling Boy's Basketball, the player must be a resident of the city of Andover or attending an Andover school. 
    
2. PLAYER'S BIRTH CERTIFICATE
   Bring a copy of the player's birth certificate to try-outs.
    
3. MEDICAL CONSENT FORM
   Bring a completed and signed Medical Consent Form to try-outs. 
    
4. CONCUSSION AWARENESS FORM
   Please read and bring a signed copy of the Concussion Awareness form to try-outs. This must be signed by both the player and the parent.
    
5. UNIFORM ORDER FORM
   New 4th Grade Players and 5-8th Grader's needing new uniform sizes need to complete the Uniform Order Form and bring to try-outs.  Also fill out this form for any player/parent who wishes to order optional ATBB apparel. (Plan to arrive at least 30 minutes early to choose uniform sizes, samples will be available.) 

   THIS WILL BE THE ONLY TIME TO ORDER UNIFORMS AND APPAREL.  Payment MUST be collected at try-outs, as it will allow us to process the orders quicker.  Payments will be held until teams are formed.
    

6. TRY-OUT & UNIFORM FEES
   Please bring the $15 try-out fee and a check for uniforms (if applicable) to try-outs. Checks can be made payable to ATBB.
